Appendix No. 2. Application for Tender Participation

Dear Ladies and Gentlemen!

JSC 'Nurbank' announces an open tender for conducting the annual PCI DSS compliance audit and invites potential suppliers to participate in the open tender, which will take place on July 21, 2016, at 2:30 PM at the JSC 'Nurbank' building, located at 10 "B" Abay Ave., Almaty, office 310.

Eligible to participate in the tender are legal entities (potential service providers) that, at the time of the tender, must meet the following requirements:

possess the professional competence and work experience, and have the necessary financial, material, and labor resources to fulfill obligations in accordance with the service agreement;
have no claims from JSC 'Nurbank' under previously concluded agreements;
be solvent, not subject to liquidation, no arrest should be imposed on their property, and their financial and economic activities should not be suspended in accordance with the legislation of the Republic of Kazakhstan;
fulfill their tax and other mandatory payments to the budget obligations at the time of application for the tender and at the time of entering into the service agreement;

The technical specification and description of services are provided in Appendix No. 5.

Tender application evaluation criteria:

compliance of the potential supplier with the above requirements;
compliance of the tender application with the technical specifications of the tender documentation;
proposed price;
payment terms;

To participate in the tender, the following document packages must be provided:
Package 1: Legal documentation
1) tender participation application;
2) notarized copies of foundation documents, certificate of registration/re-registration of a legal entity (Charter, certificate of registration for VAT purposes);
3) license copies (original for verification) for conducting designated state-licensed activities (if the acquired goods/works/services are subject to licensing);
4) notarized copy of the identity document of the authorized representative of the supplier;
5) supplier's power of attorney to the authorized representative for tender participation;
Package 2: Financial documentation

1) price proposals;
2) completed legal entity questionnaires according to the internal control rules for countering legalization (money laundering) and terrorism financing at JSC 'Nurbank';
3) original certificate from the servicing bank confirming the banking details of the applicant, the absence of overdue debt to the bank;
4) copies of financial statements for legal entities for one year, stamped by the legal entity, tax declarations for individual entrepreneurs, stamped if available;
5) original certificate from the Tax Committee at the place of registration of the supplier on the absence of tax debt;
6) copies of recommendations and reviews from legal entities/individual entrepreneurs previously cooperating with the supplier, containing assessments and characteristics of cooperation experience including timeliness, completeness of obligations, and others (if available);
7) information on the availability of material and technical base;
Package 3: Miscellaneous documentation

1) technical specifications, service provision conditions, terms.

2) Provide recommendations for major successful projects in the Republic of Kazakhstan if available.
3) The supply must be carried out once and in full volume at the customer's address.
4) The Potential Supplier bears all expenses related to the preparation and submission of their tender application, and the Bank and the tender commission are not responsible or liable for these expenses, regardless of the nature of the tender and its outcomes.

We note that the Bank's Tender Commission reserves the right not to disclose information regarding the review, evaluation, and comparison of tender applications to potential service providers or any other person not officially involved in the review, evaluation, and comparison of tender applications.

JSC 'Nurbank' reserves the right to cancel or reschedule the tender with written notification to potential suppliers who submitted tender applications about the cancellation or rescheduling, indicating the new date of the tender.

The potential supplier has the right to amend or withdraw their application before the deadline for submitting tender applications.

The tender will take place with the participation of an authorized representative of the potential service provider or without, if the authorized person does not participate in the tender.
The deadline for submitting tender documentation is 12:00 PM on July 20, 2016.
