1. Date of the organization's decision to conduct the transaction - September 29, 2015;
2. Value of the property involved in the transaction – not applicable;
3. Date of the last assessment of the property involved in the transaction – not applicable;
4. Percentage ratio of the value of the property involved in the transaction to the total value of the organization’s assets – not applicable;
5. Primary substantial terms of the transaction – an application to join the Banking Services Agreement;
6. Date of the transaction – October 22, 2015.
