Appendix No. 3. Questionnaire

Dear Ladies and Gentlemen,

JSC "Nurbank" announces an open tender for the acquisition of a Modular Uninterruptible Power Supply System for the Head Office and invites potential suppliers to participate in the equipment supply tender to be held on April 10, 2014 at 9:30 AM at the JSC "Nurbank" building, located at 51/53 Abay Khan Avenue, 5th floor, large conference hall, Almaty.

Tender applications sealed in envelopes should be submitted to JSC "Nurbank" at the address: 51/53 Abay Khan Avenue, Almaty.
Potential service suppliers meeting the criteria set forth in Appendix No. 4 of this letter are eligible to participate in the tender.

Start of bidding: April 10, 2014, 09:30 AM, address: 51–53 Abay Khan Avenue, 5th floor, large conference hall;

End of bidding: April 10, 2014, 12:00 PM;

Submission of proposals by: April 9, 2014, 12:00 PM, address: 51–53 Abay Khan Avenue

The following document packages are required for tender participation:

Package 1: Legal Documentation

1) Tender participation application in the established form, according to Appendix No. 2;

2) Notarized copies of founding documents, registration/re-registration certificate of the legal entity (Charter, VAT registration certificate);

3) Copies of licenses (original for verification) for state-licensed activities (if the activity concerning the purchased goods/services requires licensing);

4) Notarized copy of the identification document of the authorized representative of the supplier;

5) Power of attorney from the supplier to the authorized representative for participation in the Tender;

Package 2: Financial Documentation

1) Completed legal entity questionnaires according to the internal control rules aimed at combating money laundering and terrorism financing at JSC "Nurbank";

2) Original bank statement confirming the applicant's banking details, absence of overdue debts with the bank;

3) Copies of financial statements for one year, stamped by the legal entity, tax declarations for individual entrepreneurs, stamped if available;

4) Original certificate from the Tax Committee at the supplier's registration place confirming no tax debts;

5) Copies of recommendations and reviews from legal entities/individual entrepreneurs who previously collaborated with the supplier, containing evaluations and characteristics of cooperation experiences including timeliness, completeness of obligations;

6) Details of material and technical base availability;

7) Price proposals;

8) Information on the applicant's registration on ETP (if available);

9) Copies of Corporate Income Tax Declarations for two years preceding the Tender. In case of losses in tax accounting for the previous two years, the participant must justify the price of the proposed goods/service (for purchases that subsequently become subjects of international business operations).

Package 3: Other Documentation

1) Technical specifications, delivery terms, warranty service conditions, terms, preliminary contract.
